Why we ask

The claim form works differently for a child or someone else, and the next steps are different after a death.

For your claim

What to have handy

You don't need any of this to send the form or to talk to us. A claim can be lodged without everything, too: the insurer will ask for what's missing.

  • The police event numberCall the Police Assistance Line on 131 444 if you don't have it.
  • A Certificate of FitnessFrom your treating doctor, usually a GP.
  • The accident detailsThe date, the time, the exact place and what happened.
  • The vehicles involvedIncluding number plates, especially the vehicle most at fault.
  • Hospital or ambulance detailsIf you went to hospital.
  • Proof of incomeIf you're claiming lost income: pay slips from before and after the accident.
  • Details of any earlier injuryTo the same part of the body. The claim form asks.
  • A MyServiceNSW accountIf the claim will be lodged online.

Medicare, licence and bank details go only on the claim form, never to us. We never ask for them, or for a tax file number or any password.

Close to 28 days?

If your accident was close to 28 days ago and your claim isn't in yet, don't wait for our call. Call us now so we can help get your claim in on time. Claiming within 28 days generally means weekly payments can be back-paid to the day after the accident.

Your privacy

We collect your details to call you back and help with your CTP claim. We share them only with a provider you agree to, with the service providers that run our website, forms and email (some may store or process information outside Australia), or where the law requires.

If you start the form, your answers are kept in your browser on this device, not on our servers, for up to 7 days or until you send them. Your name and contact details are kept only until you close the tab. You can clear them any time with “Clear my answers”.
